PLEASE READ THE UPDATED TERMS AND CONDITIONS RELATED TO PUBLIC HEALTH & SAFTEY (COVID-19) AGREEMENT: In consideration of my participation in the sponsored activities of The Lax Box/Body Mechanics PT clinic/tournament/workshops/camps/general use of facility, I, the undersigned via electronic signature, acknowledge, agree to and understand that: 1. WAIVER & RELEASE OF LIABILITY: I am fully aware of and appreciate the risks, including the risk of catastrophic injury, paralysis and even death, as well as other damages and losses, associated with participation in athletic events. I further agree on behalf of myself, my heirs, and personal representatives, that The Lax Box/Body Mechanics PT and its event hosts, sponsors, coaches, volunteers, employees, agents, officers and, directors of these organizations, shall not be liable for any injury, loss of life or other loss or damage occurring as a result of my participation in the event, or as a result of equipment that may have been provided to me for these activities. 2. MEDICAL ATTENTION: I hereby give my consent to The Lax Box/Body Mechanics PT to contact medical resources, services, and transportation in the event of a medical emergency, and/or to provide customary athletic training attention or services as warranted during my participation in an event at The Lax Box/Body Mechanics PT. 3. READINESS TO COMPETE: I will only participate in The Lax Box/Body Mechanics PT events for which I believe I am physically and psychologically prepared to compete (this includes but not limited to pre-existing injuries and conditions, unsportsmanlike conduct and the like). If known medical, physical, or psychological conditions exist that may impact readiness to compete, I will provide evidence of clearance from medical entity before participating. I am aware that The Lax Box/Body Mechanics PT has the authority to disqualify my participation if I do not meet the criteria for readiness to compete. 4. MEDIA: I give my consent for photographs or video taken of me and the participant at the facility or during an event. Media shall be used for publicity purposes: The Lax Box/Body Mechanics PT website, brochures, flyers, news, and social media sites. 5. Individual Participant Refunds. Refunds are processed if notified more than 72 hours prior to the event start date. If less than 72 hrs prior to an event start date, only partial refunds (50%) will be processed. Once the program has started, fees will not be refundable. 6. PUBLIC HEALTH & SAFETY: I certify that Participant is physically fit, have sufficiently prepared for participation in this activity, and have not been advised to not participate by a qualified medical professional. I certify that there are no health-related reasons or problems which preclude participation in this activity. Participant/Guardian certifies that in regards to the COVID-19 virus: Participant does not currently or have previously exhibited in the last 14 days, any of the signs and symptoms consistent with COVID-19 including but not limited to: -Fever, active cough, fatigue, sneezing, shortness of breath or difficulty breathing, persistent pain or tightness in chest, blush lips or face, or any other cold/flu like symptoms. -Participant has not been out of the United States or on a cruise since March 1st 2020, if yes, participant has undergone 14 days of self isolation without COVID-19 symptoms. -Participant has not had contact with any person(s) with confirmed COVID-19 by testing. -Participant has not been in contact with any person(s) that have been out of the United States since March 1st 2020. -Participant must follow facility procedures for personal hygiene and equipment sanitation before and after participating in the activity. -If the participant is not feeling well or not able to comply with preventative health and safety procedures, they will be removed from the activity and/or group and requested to leave the premises.
